Hi,

I've just looked at 2.3.28 Documentation directory, and noticed some
inconsistencies of 00-INDEX file with actual files, so I made a patch.
Hope it helps. BTW: there are still things to checks in subdirs,
for example arm/ dir doesn't have 00-INDEX.
