Once in a while I'll see the kernel message
free_one_pmd: bad directory entry 00400000
and at that point the kernel becomes completely unusable; any attempt
to execute a command results in an immediate segmentation fault. A
reboot is required to get things going again. This afternoon this happened
during the reboot fsck, which made things pretty dicey. Eventually I was
able to nurse it through the fsck and get things going again.
