Me Too(tm). Actually, it only happened once after I tried to copy a few
files off of a corrupted floppy and tried a different floppy. I see this
in the log:

VFS: Disk change detected on device fd(2,0)
Trying to lock fdc while usage count=0
Trying to lock fdc while usage count=0
floppy0: data CRC error: track 5, head 0, sector 11, size 2
floppy0: data CRC error: track 5, head 0, sector 11, size 2
end_request: I/O error, dev 02:00 (floppy), sector 190
floppy0: data CRC error: track 5, head 0, sector 11, size 2
floppy0: data CRC error: track 5, head 0, sector 11, size 2
end_request: I/O error, dev 02:00 (floppy), sector 190
floppy0: data CRC error: track 5, head 0, sector 11, size 2
floppy0: data CRC error: track 5, head 0, sector 11, size 2
end_request: I/O error, dev 02:00 (floppy), sector 190
VFS: Disk change detected on device fd(2,0)
Trying to lock fdc while usage count=0
VFS: Disk change detected on device fd(2,0)
floppy0: fractionary current count b=0 s=0
addr=0, length=0
st=0 ast=0 mse=18 msi=18
ssize=1 SIZECODE=2
command=e6 SECTOR=1 HEAD=0, TRACK=0
buffer drive=0
buffer track=-1
buffer_min=0
buffer_max=0
end_request: I/O error, dev 02:00 (floppy), sector 0
VFS: Disk change detected on device fd(2,0)
fatfs: bogus cluster size
VFS: Can't find a valid MSDOS filesystem on dev 02:00.

...I believe at the end here I was trying to mount a non-fat-formatted floppy
as -t fat, which obviously failed.
