Rename or otherwise disable:
/usr/sbin/in.telnetd
/usr/sbin/in.rlogind
/usr/sbin/in.rshd
(look in /etc/inetd.conf for others)

No, but it doesn't happen by default. Look in /etc/syslog.conf. Add an
entry for your permanently-connected terminal.
