This looks like some unckecked memory allocation failure to me.

The 'scan scsis' code does not really check for memory allocations
failure. This may be due to the facts that in the early time of Linux,
modules were not available and memory allocations are very unlikeley to
fail at system initialization. On the other hand, this code tries to
allocate ISA DMAable memory which has some not negligible probability to
fail after system initialization. The foot (since we only have 2 hands)
:-) is that the 'scan scsis' code uses GFP_ATOMIC priority for the ISA
DMAable allocations (and also for allocations that are not to be
guaranteed for ISA DMA).

Note that if we just would check for failures and do nothing on error, the
scsi scan would not work better, but may-be, this would leave some chance
to succeed by unloading and then reloading the driver.
