smc-ultra irq? problems in 2.1.13[01]

My smc ultra which has been working for over a year now has now seemed to
have died all of the sudden. The only thing that changed was going to
2.1.130, when i rebooted it wouldn't work any more and subsequently
upgrading to 2.1.131 didn't help either. I'm not so sure it is a kernel
problem, but figured I would start here.

Originally I had the smc-ultra being built into the kernel, but after the
problems started I made it a module to do some testing with other irq's.

Basically the modules for the smc load and ifconfig eth0 works without any
errors. Here is what I get after trying to use the interface however:

You can see where I unloaded the module and changed the irq, I have tried
irq 5,7,9,11,12 but none work. I was originally using irq 11 which had
always worked. Here is an output of ifconfig after trying to use the
interface:

Any one have any ideas as to what may have happened? NOTE: my wife tried
to install some dos game under windows just before this as well which had
trouble auto-detecting the PnP sound card, sound still works, but smc
doesn't.
