pppd 2.3.2 works fine for me, currently running with 2.1.76. Did you apply
the pppd-2.2.0 patch from Documentation/networking/ppp.txt ?

I use the pppd demand-dialing feature so this might be my way of avoiding
your problem - the ppp0 device always exists, and the default route points
to it.
