/usr busy on 2.1.65 reboot

I am hitting a problem with rebooting in 2.1.65.

At umount -a in rc.6 /usr is busy, forcing a fsck on restart.

/etc/mtab is:

/dev/hdb2 / ext2 rw 1 1
/dev/hda3 /usr ext2 rw 1 2
/dev/hda4 /usr/local ext2 rw 1 2
/dev/hda1 /DOS msdos rw,umask=000 0 0
/dev/hda5 /DOSE msdos rw,umask=000 0 0
/dev/hdb1 /DOSD msdos rw,umask=000 0 0
/dev/hdb5 /DOSF msdos rw,umask=000 0 0
none /proc proc rw 1 0

It doesn't happen every time, but I don't know what is causing it.
