I've been having the problem in a system with 2 IBM SCSI disks:

Host: scsi0 Channel: 00 Id: 00 Lun: 00
Vendor: IBM Model: DALS-3540 !s Rev: S60E

Host: scsi0 Channel: 00 Id: 01 Lun: 00

But I have enough of these in other systems with no problems that I really
don't believe they're responsible. I'm much more suspicious of the cheap
motherboard and non-parity RAM in this box. Nearly all the others are
Tyan boards with true parity RAM.
