1.3.88 OOPS & bugs in ksymoops

I just had 1.3.88 OOPS on me, while mounting a cd-rom. While
trying to ksymoops it, ksymoops just seg faulted. So here is the
original oops log and my hand-edited symbol resolution.

I have an sbpcd-style cd drive, and was doing a tray open mount.
(CD tray open when mount command issued.) I can replicate it in every
1 in ~20 such mounts.
