I have tried this, and not managed to recreate this. I have a similar
system.

This is probably due to the naff design of the CMD chip, what speed is your
bus running at. I have had to reduce mine to 1/2 clock speed (20 Mhz) with
one wait state (needed for the graphics card, so don't know about the disk
I/O) when I added a new drive in order to keep the system stable. At
40Mhz it wouldn't even boot most of the time. If you can slow down your
bus it might help.
