Last night I had my machine fall over with the same problem.

It is a DX2/66 running a standard 1.3.10 kernel.

It also produced this message

Jul 21 19:27:02 enterprise kernel: Warning: bad magic number for tty struct
(4, 5) in tty_ioctl

Here is the oops

The machine then fell over when the swapper died.
