I have an offhand guess that the following sequence might reproduce it:

<write data to a fsdax file1>

<process 1 maps file1, which sets up the pagecache mapping to pmem, and
goes to sleep>

cp --reflink=always file1 file2

<process 2 maps file2, which tries to map the same pmem "page" into
file2's pagecache and trips over page->mapping already being set to
file1's pagecache>

But I dunno, I haven't had much time for digging into this one.
