[PATCH 23 of 36] x86: unify pmd_pfn

From: Jeremy Fitzhardinge
Date: Thu Feb 05 2009 - 14:39:27 EST

Impact: cleanup

Unify pmd_pfn. Unfortunately it can't be demacroed because it has a
cyclic dependency on linux/mm.h:page_to_nid().
