[2.6 patch] drivers/block/paride/ cleanups

The patch below does the following cleanups in each if the five changed
C files:
- #ifndef MODULE: remove unused setup function
- make a needlessly global struct static

After this cleanup, setup.h is completely unuse and therefore removed.
