I'm just adapting references, not rewriting and correcting the text.

Consistently reference files in Documentation/:
- remove preceding "../../", "linux/" and "/usr/src/linux/"
from the "Documentation/blah.txt" string.
- point to new locations if files have moved
However, there are some special cases where the documentation file is
not located in Documentation/ even if they are referenced as being there:
README.epca:
Probably formerly called "user.doc". Now located in drivers/char/README.epca.
Strange reference as "Documentation/README.epca.dir/user.doc" in
Documentation/digiepca.txt.
New reference is: drivers/char/README.epca
HFS.txt:
Referenced as "Documentation/filesystems/hfs.txt" in
fs/hfs/FAQ.txt. Real location is "fs/hfs/HFS.txt". New reference
adapted accordingly.
