Getting wierd error message on machine which eventually crashes

Hi all,

    One of my Linux systems has crashed twice and when reviewing the logs I
found the following error messages within /var/log/syslog. The same error
messages also showed up before the first crash. The first crash occured
under 2.2.14 and this one occured under 2.2.15. The machine is an Athlon
550, 256MB memory, and has a RAID 5 setup with the ICP-Vortex controller.
What would cause this message? Is it a hardware issue or kernel one?
