pmd errors

I get a lot of following errors, followed by random crashes and
kernel oopses.

On 2.2.x kernels i get:
Apr 19 11:16:57 rusty kernel: free_one_pmd: bad directory entry 00008000
Apr 20 13:52:06 rusty kernel: swap_free: swap-space map bad (entry 00008000)
Apr 20 13:52:12 rusty kernel: swap_duplicate at c01236e2: entry 00008000, unused page

On 2.3.99pre kernels i get:
Apr 17 10:51:23 rusty kernel: memory.c:83: bad pmd 00008000.

What could be the cause of this errors? i assume its swap space, any ideas?
